In order to obtain FOP logo tags, you must be:

• Be an Active, Active Retired or Associate member of FOP Lodge; AND 
• Be the owner of co-owner of the vehicle/motorcycle

REQUIRED DOCUMENTS: Please contact the Lodge for the application - application must be completed in-person. 

Members MUST provide the following in-person or by mail:

• Driver's license
• Vehicle registration or bill or sale 
• Proof of insurance
